Thanks so much for your interest in using the Grace Bible Church Facility. It is our hope and desire to make every request happen but in some cases we need to make adjustments. Please fill out as much detail as you can. Details help us better serve you and your event. We accept facility requests for up to 6 months in advance. Certificate of Liability Insurance

Outside organizations requesting the use of the facility must present a Certificate of Commercial General Liability Insurance in the amount of $1,000,000 that names Grace Bible Church as an "additional insured". This insurance must include coverage for Improper Sexual Conduct in the amount of $1,000,000 per occurrence. Also, an ADDITIONAL INSURED ENDORSEMENT is required as a verification of the certificate holder for commercial General Liability & Improper Sexual misconduct.

Hold Harmless and Indemnification Agreement: *

To the fullest extent permitted by the law, I (the person or representative of the group named above) hold harmless and defend Grace Bible Church, its employees and agents from and against all claims, demands, suits, damages, judgement of sums of money, losses, and expenses, including but not limited to attorney's fees and costs arising out of the performance of any of the services to be performed pursuant to the terms of this Agreement, provided that any such claims are attributable to bodily injury, sickness, disease or death, or to injury to or destruction of tangible property including the loss of use resulting there from; and are caused in whole or in part by any act or omission of myself, or the group listed above, regardless of whether or not they are caused in part by a party indemnified hereunder.
